<p>Many of our readers report that Ecuador health care is so inexpensive that they do not need health insurance.   Because of this Ecuador health insurance is not expensive.</p>
<p>There are many ways to be healthier in Ecuador.  Many ancient healing practices are regularly used.  Here is Steve with two shamans at a healing spring near Cotacachi.</p>

<p>For those aged 45 to 64 Ecuador health insurance costs for unlimited coverage is about $30 a month. Medicines, albeit generic, are free.</p>
<p>Ecuador health insurance for those aged 65 or over rises to $75 per month.</p>
<p>This Ecuador health insurance is offered by a company new to Ecuador but it has been established in Colombia since 1969. This company is the largest health insurance company in that country of 44 million people. It is the 10th largest company of any kind in that country and has a work force that numbers more than 15,000 employees. In short it&#8217;s a very well established health insurance company.</p>
<p>It is easier to get fresh food in Ecuador, a land of year round abundance. Here is a typical market scene.</p>

<p>Non Ecuadorians are accepted for Ecuador health insurance by simply showing their passport as identification.  At this moment from my conversation this afternoon it is not clear whether you need to be a resident in Ecuador or not. The sales manager I spoke to suggested it is not necessary but I would like to see written confirmation.</p>
<p>Folks 65 or older need to take out a pre-acceptance exam that focuses on brain, bone and cardiac health as well as blood, urine and faeces tests.</p>
<p>I asked if pre-existing conditions are covered. The answer was, &#8220;yes if not too serious&#8221;, and certain restrictions apply.</p>
<p>For example, somebody with elevated cholesterol levels would be accepted into this Ecuador health insurance but somebody who has cholesterol levels shooting through the roof would not be accepted. In other words this is not a catch all health insurance scheme but it does offer a good choice for those with below average through to good health.</p>
<p>Two days after taking out this Ecuador health insurance, you can obtain at anyone of 5 centers in Ecuador, dentistry, x-rays, laboratory tests, ambulance services and free medicines from the pharmacy. After 1 month hospitalization is covered and after 3 months surgery is covered.</p>
<p>I asked about cancer, heart attacks and strokes. Much to my surprise, all are covered without any ceiling in costs. One hundred percent of the cost is covered. You even don&#8217;t have to pay up front and then go through hassle to get the money paid back.</p>
<p>Living in Quito or Guayaquil, Santo Domingo, Machala or Riobamba, coverage is complete. If you live in Cotacachi or elsewhere you are entitled to coverage in local hospitals if you suffer any kind of emergency or accident. For routine checkups or non-emergency treatment you would need to travel to a clinic in one of the above cities.</p>

<p>  Turns out that Juan has been here in Cotacachi for 3 years married to a local lady but originally he is from the city of Cajamarca in Peru. There he learnt from his grandfather how to prepare what I call the Aloe Vera drink concoction &#8211; otherwise known as sabila here in Cotacachi.</p>
<p>  His previous customer urges me &#8220;drink it, drink it, it&#8217;s really good for you&#8221;. How could I say no?When in Cotacachi do as the Cotacacheños do.</p>

<p>Here in Cotacachi he gets up earlier than I care to contemplate to be on the road and selling to early risers at 5am. By midday he&#8217;s all done.</p>
<p>Juan buys the aloe vera in Cotacachi but it&#8217;s shipped up from the sub-tropical Intag valley just 40 miles away. He buys local honey from the market in Otavalo.</p>

<p>Here&#8217;s how he starts, he separates the aloe vera from the green plant using a knife and a rapid scraping action. After he has a fair amount in the bottom of a jug he adds some water. Honey too.</p>

<p>  Add to that crushed linseeds too. It&#8217;s a pretty viscous solution by now but Juan has a practiced technique &#8211; pour it from one jar to another to somehow make it less gooey. </p>

<p>  Just for good measure the stone-breaker plant is added. While Juan is at it he also adds Boldo, a Chilean plant.</p>
<p>  All the plants and herbs reputedly promote good intestinal, liver, gall bladder and kidney well-being. Here is what Wikipedia says about the stone-breaker plant called chanca piedra in Spanish.</p>
<p>&#8220;The plant has long been used as an herbal remedy for urinary calculi, and has been shown in modern medical research studies to reduce the risks of stones in individuals prone to the condition. Research on the plant continues to determine if it has any other beneficial effects. It may have antiviral activity.&#8221;</p>
<p>And about Boldo .. &#8220;..In Latin America and Spain, boldo is also used as a form of herbal medicine, particularly to support the gallbladder, but also to calm upset stomachs. In Chile, it is frequently mixed with yerba mate or other teas to moderate its flavor. In Brazil, many families keep a boldo plant at home for this purpose, although boldo teabags are readily available in nearly all supermarkets.&#8221;</p>
<p>  Then to serve this concoction in a more palliative form Juan heats it using a liitle gas fired burner hidden away inside his &#8216;seen-better-days&#8217; mobile serving trolley.</p>

<p>And here&#8217;s what I say!! It&#8217;s surprisingly not bad! The taste is ok and it kind of slimes down your throat quickly enough that I didn&#8217;t feel any ill affects. </p>

<p>This is cheap preventative medicine at 50 cents a glass and if it&#8217;s effective then who can dispute the value? I&#8217;m not sure I want to drink it every day but plenty of folks do and who in the end would argue against the wisdom of the ages handed down from Andean grandfathers for the benefit of all who live in Cotacachi.</p>

<p>Mariana studied biology and chemistry at the nearby University of Ibarra when she was 20 before moving on to the central university in Quito where she studied general orthodontics.</p>
<p>She had to spend a year in a rural location before she could receive her professional qualification but she was lucky in as much as she spent 6 months in the nearby parish of San Roque and then 6 months in Cotacachi hospital.</p>

<p>Immediately afterwards she opened her own surgery on the main commercial street of Cotacachi, Avenida 10 de Agosto. That was in 1987 and she has been here ever since. Twenty one years service to the community. That makes her an experienced dentist in Cotacachi .. or anywhere.</p>
<p>Well not quite 21 years of service.  She did also spend 8 years (2 terms) as a local councillor. She says that being an orthodontist is a great career for a woman because you can work from home and you get to meet all the local community. <p>She is still involved in social action and now is trying to set up a shelter for women and children who have been abandoned, with the aim of also teaching the women a new skill so they can be independent.</p>
<p>Still, this experienced dentist in Cotacachi still loves her profession after all these years. She has a huge smile for all her patients &#8230;</p>

<p>A porcelain crown would be $110. A complete set of dentures using poli-chroming would set you back $390. A cleaning of all teeth is $15 and a whitening using regular technique is just $40. Her hours of opening are 8am-1pm and 2.30pm &#8211; 7pm.</p>

<p>The hospital was founded in 1977 as a county hospital similar to others all over Ecuador. Since then it has expanded physically and also offers more services. The man in charge is Doctor Oscar Castillo.</p>

<p>Cotacachi county is divided into 14 parishes and some of these parishes are 80 miles away 7,000 feet lower in a tropical environment. Different illnesses down there for sure that don&#8217;t even exist in the mountains. Each parish has one or two health centers. Dr. Castillo has quite a responsibilty but standing over 6 foot tall and with a ramrod back he does exude an air of quiet authority.</p>
<p>Cotacachi hospital has both a vision and mission statement. The vision statement is telling of the ambition and pride everyone attached to the hospital feels.</p>
<p>&#8220;To convert Cotacachi into a healthy county in which the multi-ethnic peoples live in social, spiritual, material and environmental harmony exercising shared rights and responsabilities in a participative democracy applying principles and values of equality and solidarity prioritizing the promotion of health with universal access to health services of warmth and quality using both western and traditional knowledge and practices.&#8221;</p>
<p>Cotacachi hospital is funded by the Ministry of Health based in Quito and as such ALL services are free. This is quite remarkable in a supposedly &#8220;third world country&#8221; but the reality is that Ecuador better fits the description of an &#8220;emerging country&#8221;.</p>
<p>There are doctors providing consultations to the local citizens, a specialist childrens doctor. Services include vaccinations, pregnancy care and general medical practice.</p>
<p>There are four emergency doctors who treat burns and injuries from accidents and also an operating theater with a general surgeon.</p>

<p>Cotacachi hospital does not treat broken bones nor patients who&#8217;ve suffered a heart attack or stroke. These patients are rushed by ambulance to city hospitals or clinics in nearby Ibarra. Or they are brought to Cotacachi hospital to be stabilzed before being transferred to other clinics.<br />
The ambulance service is free too including transfer to other hospitals or clinics, even as far away as Quito (75 miles).</p>

<p>I seemingly have some issues with my liver and small intestine. The following day after that test Doctor Vaca came to visit me in Meson de las Flores again. <p>We went up to our Serendipity room at El Meson which is used for masages and infra-red saunas and now Doctor Vaca treatments! Out of his kit bag came a machine called a Cell Spa. Apparently they are relatively common in the USA but I hadn&#8217;t seen one before.</p>

<p>They are used with an oscillator, mineral salts and a foot bath. Here&#8217;s what happens:- You put your feet in the clear transparent water. You sit back and listen to relaxing music while the machine works for 30 minutes. Meanwhile the opposite of osmosis is taking place.</p>

<p>Our feet are able to emit toxins and so the machine is literally sucking the toxins out of my body. The good Doctor has all the instructions in Spanish. If you remember from yesterday&#8217;s article my liver showed up as having some alterations and lack of functionality. In Spanish, the word for liver is &#8216;higado&#8217;.</p>

<p>So the experience is pleasant enough. But look at the results! Look at the color of the water now. It&#8217;s brown with an oily film on top and a brown residue on the bath at the level of the water.</p>

<p>The activity of the various acupuncture points (and therefore the activity of the meridian and health of the related organ) is defined by the pain sensitivity at the place of the acupuncture point location. <p>My readings showed up a disturbing lack of functionality in the liver, probably attributable to too much coffee drinking and not enough exercise. The coffee I will only very reluctantly give up but living in Cotacachi there is no excuse not to get out in the freshest air you&#8217;ll encounter anywhere and walk the miles of country lanes that run out to little indigenous communities. <p>  Indeed, not much medical information is available from Ecuador or other Latin countries. Yet, just because not much information is available that most definitely does NOT mean that there is a lack of health infrastructure in Ecuador or Cotacachi and nearby towns and cities.</p>
<p>  In El Meson we have a list of dentists and doctors who we can call up and make an appointment on your behalf for a consultation or treatment. Most of the treatments are many many times cheaper (maybe the average is six times cheaper) than at home and some are surprisingly alternative. They just not are easily available at home.</p>
<p>  No Cotacachi medical information would be complete with out introducing the doctors and dentists who provide Cotacachi healthcare. My favorite is Doctor Vaca. With good reason I call him the &#8220;laughing doctor&#8221;. I have <em>never</em> seen him carry out his work in state of mind other than good humor.</p>
<p>  I first heard of this when our long-time friend and reader, Micky Enright, accompanied him to the local old people&#8217;s home run by the local nuns. There he showed boundless compassion and enthusiasm as he entered the main lounge by actually taking a spin on an impromptu dance floor with all the ladies who were able to stand and move a little.</p>

<p>  Doctor Vaca has also treated patients at El Meson too. In one case, someone who surely should not have travelled down here had a pre-existing condition that took a turn for the worse. I translated between the doctor and the patient and as the patient vomited blood, to my un-trained eyes I believed he might even die.</p>
<p>  But Doctor Vaca calmly went about analysing and then treating our client. The care was very professional and caring  and the patient was stabilised using drugs administered via a drip. He then came back three times a day to check on the patient&#8217;s progress and was always just a cell phone call away.</p>
<p>  There was no hospitalization and I believe the final bill including all medicines was less than $60. For the patient this was a very unfortunate condition he was accustomed to living with but his wife was extremely concerned. Needless to say after the patient stabilized his wife was immensely grateful and relieved.</p>
<p>  Doctor Vaca was born in Cotacachi in 1952. When he was 19 he took the very unusual turn of heading off to was was then the Soviet Union and now is the Ukraine to study medicine. This has had a profound influence on his subsequent believes because although he studied to be a conventional doctor his studies exposed him to research and education in camps such as energy medicine.</p>
<p>  His first year was spent on preparatory studies and learning Russian in the city of Kharkov. Then in 1972 he began six years of studies at the A.A. Bogomolets National Medical University, named after one of Russia&#8217;s most famous pathologists.</p>

<p> When he returned to Ecuador Doctor Vaca, in common with all other Ecuadorian junior doctors, had to give a year&#8217;s service in a rural community. At that time, Cotacachi was a rural community and in fact I know another dentist from Quito who was sent to Cotacachi but Doctor Vaca was instead sent in 1978 to a small community deep in the Amazon rain forest!</p>
<p>  After a year there he went to the southern city of Loja in order for his Russian medical title to be re-evaluated so he could receive his Ecuadorian license to practice. Then he had to serve a residence term. This was done in the government clinic at Atuntaqui, a small town just 20 minutes away from Cotacachi. Spep by step, year by year, he was getting closer to his beloved Cotacachi.</p>
<p>  Finally, in 1982, 11 years after travelling as a 19 year old youth to Ukraine he was able to start his own private medical practice here in Cotacachi. Since then Doctor Fabian Vaca has been worked from his own consulting office in Cotacachi and also provided private medical care at the Cotacachi&#8217;s old folks home, to 100+ workers at a rose plantation and also the workers at a nearby turkey farm.</p>

<p>Doctor Vaca hasn&#8217;t forgotten about helping the community and he still travels once a week to treat an indigenous community on the shores of Lake Mojanda at 10,000 feet.</p>
<p>  Doctor Vaca is a wealth of Cotacachi medical information and much of that has been acquired providing public service as well as private practice. From 1994 to 2002 he worked at Cotacachi hospital and was the Director of the hospital for 4 years. He was also the director of the hospital at Otavalo between 2002 and 2005 where he specialised in peadiatrics.</p>
<p>  Dr.Vaca still has contacts with his student friends in Russia. This was how he was able to acquire software and machinery from a Russian organization that works using acupuncture points and variable thermo-algometry to detect dysfunctions of internal organs and systems and weak parts of regulation.</p>

			<content:encoded><![CDATA[<p><strong>Cotacachi medical information is scarce but that doesn&#8217;t mean the medical facilities are scarce.</strong></p>
<p>Watchout in upcoming posts for a more detailed analysis of what is available but today let us take an overview of what is available in this charming village.</p>
<p>  For a small rural village of 5,000 people there are a number of doctors and dentists, a community hospital, alternative health center, 2 clinical laboratories for testing samples of blood and urine etc., and also a hard to track down (but I&#8217;m working on it) French chiropractor.</p>
<p> Below is the hospital which is on Calle Pedro Moncayo just slightly out of the center of the town. <p>And below is a dentist, not the one we normally recommend but another, who has her small dental clinic in 10 de Agosto as well. She charges NOTHING for a checkup and $10 for a cleaning or $40 for a whitening which will last for 30 months unless you drink a lot of coffee she tells me laughingly &#8211; she must be a psychic! How&#8217;s that for useful Cotacachi medical information?</p>

<p>And here is the home/consulting office of our good friend Doctor Vaca who manages to combine general medicine with Russian energy medicine, he trained as a doctor and graduated in the former Soviet Union but now part of Ukraine.</p>

<p>Just yesterday Dr. Vaca ran me through his Russian energy medicine software program. The results will remain largely private but suffice to say they were amazingly accurate and can also serve as very timely preventative medicine. More on that in another post.</p>
